In order to register your claim with Cello Aviation we ask you to complete the following form and provide scans of your boarding passes and your tour operator invoice.  If you are unable to scan these documents you can send the documents or copies by post to: Customer Services, Cello Aviation Ltd, 140, Holyhead Rd, Birmingham, B21 0AF, UK.

When you complete the below document it will be transmitted by email and stored in our secure servers.  If you have any concerns about sending personal information by email then we ask you send it to by post to Customer Services at the above address.

There may be a short delay once you have submitted the form, an acknowledgement will show at the top of this webpage and you will also shortly receive an automated email confirming receipt.
